First Responder Therapy

Compassionate support for those who show up for everyone else.

The work you do is intense. Long shifts. High stakes. Constant alertness. Whether you’re responding to emergencies, witnessing trauma, or carrying the weight of other people’s worst days, it takes a toll — on your mind, your body, your relationships, and your sense of self.

At Avena, we offer therapy built specifically for first responders — no fluff, no judgment, no explaining what the job is like. We get the culture, the pressure, the silence, and the stigma. And we create space for you to offload, process, and breathe.

This isn’t about “fixing” you — it’s about helping you make sense of what you’ve been through and what you’re still carrying. Together, we’ll sort through the stress, the exhaustion, and the moments that stick with you long after the call ends.

Therapy here is confidential, real, and grounded in the belief that strong doesn’t have to mean silent. You’ve spent your life showing up for others — we’re here to help you show up for yourself.

Here's what we'll work on in therapy:

$

Processing the critical incidents, close calls, and images that don’t leave easily

$

Learning how to turn off “work mode” and reconnect with yourself and those you love

$

Navigating burnout, compassion fatigue, and the slow creep of emotional numbness

$

Naming the stress you’ve been minimizing — and understanding how it’s showing up in your body and behavior

$

Building tools to manage hypervigilance, sleep issues, and constant adrenaline spikes

$

Reclaiming a sense of purpose, balance, and control outside the uniform

Do you offer therapy specifically for first responders (EMS, fire, police)?

Yes. Our team has specialized experience working with EMS, fire, and police professionals. We understand the culture, the pressures, and the specific challenges that come with first responder work, and we tailor treatment accordingly rather than relying on a one-size-fits-all approach.

How do you treat PTSD and cumulative trauma from the job?

Treatment is individualized based on your history and current symptoms. Common approaches include:

  • Evidence-based trauma processing techniques
  • Coping strategies for intrusive memories and hypervigilance
  • Support for cumulative or “layered” trauma built up over a career, not just single incidents

Is this therapy confidential and separate from my employer?

Confidentiality is a cornerstone of our practice. Your sessions, records, and the fact that you’re in treatment are protected and are not shared with your employer or agency without your explicit written consent, except in rare legally mandated circumstances.

Do you understand shift work and can you accommodate scheduling?

We know first responder schedules don’t look like a typical 9-to-5. Our team offers:

  • Flexible appointment times, including early morning and evening slots
  • Telehealth options that work around rotating shifts
  • Openings for new patients within one week

Do you treat burnout, compassion fatigue, and critical incident stress?

Yes, these are among the most common concerns we see in first responder clients. Whether you're dealing with chronic burnout, compassion fatigue from repeated exposure to others' trauma, or the acute stress of a specific critical incident, we offer targeted support for each.

Do you accept insurance or union/EAP benefits?

We accept insurance from Aetna, Blue Cross Blue Shield, and United Healthcare (which also covers Optum, Oscar, Oxford, and UMR plans). We also work with private pay clients. If you have union or EAP benefits, reach out to our team and we can help clarify what's available to you.

Do you offer telehealth for first responders who can't easily get to an office?

Absolutely. Telehealth is one of the most popular options among our first responder clients, since it removes the barrier of commuting on a limited or unpredictable schedule. Sessions are available via video or phone, whatever fits your routine best.

Real People. Real Expertise. Real Change.

At Avena Psychological Services, we offer holistic, individualized therapy designed to help you understand yourself more deeply and create meaningful, lasting change. Our licensed psychologists tailor each session to your specific concerns and goals, drawing from your full life experience—including your childhood, culture, relationships, physical health, and personality. Whether you’re struggling with depression, relationship challenges, or feeling stuck in patterns that no longer serve you, therapy can help you break your issues down into manageable pieces and begin moving forward. Left unaddressed, mental health concerns can impact every area of life—from work and school to physical health and relationships. But with the right support, you can shift your mindset, make empowered choices, and experience real transformation.

Frequently Asked Questions

What conditions do you commonly treat in first responders?

We frequently work with PTSD, anxiety, depression, burnout, compassion fatigue, substance use concerns, and relationship strain related to the demands of first responder work.

Will my employer or agency ever be notified I'm in therapy?

No. Your participation in therapy is confidential and will not be disclosed to your employer or agency without your written permission, barring rare legal exceptions such as imminent safety concerns.

Do you offer group therapy specifically for first responders?

Group therapy availability can vary. Contact our team directly to ask about current group options and whether a first responder–specific group is currently running.

How soon after a critical incident should someone seek therapy?

There’s no strict timeline, but reaching out sooner rather than later can help prevent symptoms from becoming more entrenched. Even a single session shortly after an incident can provide valuable support and coping tools.
